Applicants Must Submit the Following:
  1. City of Norfolk Public Safety Application for Employment.
  2. Photocopies of birth certificate, high school diploma or GED certificate, driver's license, and DD214 (long form), if applicant ever served in the military.
  3. Attach copy of Department of Motor Vehicle Report (not over 1 year old) from state being licensed.
  4. Notarized copy of Authorization for Release of Information form.
Background documents that also must be submitted (if applicable):  
  1. Copy of certificate or college transcript.
  2. Copy of marriage license.
  3. Copy of divorce papers
  4. Copy of other training certificates.
