In an effort to continually provide outstanding customer service, the Division of Parking is providing its Monthly Vehicle Parking Rental Agreement online.  Customers are required to complete a form providing basic information regarding their vehicle, contact information and billing information.  Please click here to download the new Monthly Vehicle Parking Rental Agreement form in PDF format^*.

Please click here to print the Monthly Vehicle Parking Rental Agreement.  Once you have read the entire Agreement, initial and sign the Agreement in the designated areas.  Bring the Monthly Vehicle Parking Rental Agreement to the Parking Customer Service Center at:

Once a contract has been signed and approved, monthly customers will receive an individually-coded access card and/or numbered tag to display from their vehicle's rearview mirror.

*     By signing this Monthly Vehicle Parking Rental Agreement, monthly patrons are acknowledging that they have read and understand the Monthly Vehicle Parking Rental Agreement.  Furthermore, the customer agrees to and accepts all the terms and conditions thereof.
