Vital Records

Norfolk Department of Public Health Vital Records offers a wide range of services to customers. The office of Vital Records is the city’s official records keeper for vital information pertaining to acknowledgement of paternity, minor name changes and home births occurring in Norfolk.

The Vital Records office is authorized by state statute to charge fees for searching for vital records and for other services including providing certified copies of death records.

The Vital Records office provides for the registration, correction and certification of deaths and fetal deaths. By law, only certain persons are entitled to copies of vital records.

We are committed to meeting our customer’s needs and expectations with quality service.

Vital Records Death Certificate

You are entitled to a certified copy of a death certificate if you are a member of the decedent’s family or if you are their legal representative.

Other applicants who may have a tangible interest but do not qualify for a death certification may receive a verification of death that includes the descendents name, date of death, date of birth and place of death.

Certified copies of death certificates may also be obtained from the state vital records office. Local certified copies of death certificates may also be issued from the Norfolk Department of Public Health. The Norfolk vital records office can only certify deaths that occurred during the last 5 years.

Required Information for a Death Search

  • Full name of the person whose death certificate is being requested Full date of death (month, day, year)
  • City where the person was pronounced dead
  • The names of the decedent’s parents
  • Signature of the person requesting the death certificate
  • Relationship between the person name on the death certificate and the person requesting the certified copy
  • The full name and complete mailing address of the person to whom the certificate is to be mailed
  • Area code and daytime telephone number of the person requesting the death certificate
  • Other information you consider helpful in the search for the death certificate ( Example: nicknames, alias, place of burial, etc.)

Records Search - $12.00 per death record
Death - If the record is not located, the search fee is nonrefundable.

The following information must be provided:

  1. Full name of person whose death is being requested;
  2. The full date of person birth or death
  3. The city’s where the event occurred;
  4. Relationship to descendant

The fees associated with a regular request by mail is $12.00 for each copy. The usual turnaround time for regular request by mail is 3-5 working days. Please note: Applications for Certification of Vital Records are not mailed out from the Norfolk Department of Public Health.

How can I get a birth certificate?
All birth certificates are issued from the Richmond Bureau of Vital Statistics.  Please call 804-662-6200 for more information.

What is the cost of a death certificate?
The Code of Virginia requires a cost of $12.00 per certificate/search. There is no refund if the certificate is not found. If no record is found for a death certificate request, a “No Record” letter will be issued.
Make checks or money orders payable to: Norfolk Department of Public Health. DO NOT SEND CASH.

What hours are you open?
We are open to the public from 8:15 am - 5:00 pm. Monday-Friday. We are closed on major holidays.
