WIC
Program
WIC stands for the Special Supplemental Food Program for Pregnant
and Breastfeeding Women, Infants, and Children. The purpose
of the program is to provide supplemental foods and nutrition
education to eligible persons. The program will also serve
as an adjunct to good health care by promoting healthy outcomes
of pregnancy, improved eating habits, and improved health
of women, infants, and children. Pregnant and breastfeeding
women, and mothers after delivery, even if they decide not
to breastfed, infants (birth to 12 months), and children (over
12 months to 5 years) must meet certain income guidelines
and be at nutritional risks to be eligible for WIC. The WIC
Program is funded by the United States Department of Agriculture
(U.S.D.A). The WIC program not only provides nutritious foods
such as cheese, peanut butter, beans, milk and infant formula
but also provides nutrition education and referrals to other
health care systems.

WIC Requirements
Proof of income can include any of the following:
- Current Medicaid card/HMO
- LES (Military Leave and Earning Statements)
- Pay check stubs
- Room and Board Statement(To be completed by whom you
live with. Ask the WIC office to send you the form in the
mail.)
IF NO PROOF OF INCOME IS AVAILABLE AT THE TIME YOU COME
INTO THE WIC
OFFICE, ARRANGEMENTS CAN BE MADE TO GET YOU STARTED ON THE
PROGRAM.
WIC Referral Form
For faster service, ask your doctor to complete a WIC Referral
form with the current height weight,
and blood work for each participant. REMEMBER: THE HEIGHT,
WEIGHT, AND BLOOD
WORK ON YOUR WIC REFERRAL FORM CANNOT BE OVER 60 DAYS OLD.
Bring your completed WIC Referral form to your WIC visit.
What happens if I forget my referral form for a visit?
We value you as a customer, please still come to your appointment
and bring the child or infant.
We have a Nutritionist Assistant at each site, who will do
the blood work and measure height and weight.
