What you Need
To apply for an original Disabled Parking Placard or License plate you will need
to fill out and bring in a
MED 10
form. This application must include the signature of the doctor certifying your
disability.
If you are applying for a disabled veteran's license plate you must also present
a
VSA 54
Veteran's Certificate of Disability, and the
VSA 10
aplpication for License PLates.
OR
You can bring in just the VSA 54
signed by both a medical professional and by the Veteran's Administration along with the
VSA 10
Associated Fees
Temporary and permanent placards for citizens cost $5.
How can I pay for my Placard?
Plate fees can be paid by cash, check, credit, or debit. Important:
payment forms cannot be split between credit/debit and other forms. If you plan
on paying with a card, the sum of your payment must go on the card. You may split
cash and check
Military Personnel
There are no changes for military personnel to the above information.
