Substance Abuse Services
Case Management
The mission of the Substance Abuse Case Management Program is to improve the lives of individuals with substance abuse and/or co-occurring disorders. An adjunct to treatment services, case management improves post-treatment outcomes.
Substance Abuse Case Management is designed to assist individuals and their family members in accessing services that are responsive to the person’s individual needs. Substance Abuse Case Managers will:
- identify and reach out to potential individuals receiving services;
- assess needs and plan services; link the individual to services and supports;
- assist the person directly in locating, developing, or obtaining needed services and resources;
- coordinate services with other providers; enhance community integration;
- make collateral contacts; monitor service delivery; and
- advocate for people in response to their changing needs.
These services are based on the 5 elements of case management according to the National Association of Social Workers (NASW), which are:
- Assessment
- Service planning
- Linking to services
- Advocacy
- Follow-up and ongoing monitoring
Services with SA Case Management include:
- Comprehensive assessment of case management needs
- Development of, in conjunction with the individual, an Individual Service Plan (ISP)
- Individualized education including: life skills related to independent living
- Linkage to Department of Rehabilitative Services (DRS) for employment counseling and job placement
- Use of community resources, especially recovery support group
- Transportation services to support the plan of care
- Monitoring of abstinence through drug screening
- Linkage to mental health benefits
- Assistance with applying for benefits
- Monitoring quality of service delivery and client satisfaction
- Advocacy
- Referrals for food
Case Management Umbrella
Four tailored Case Management Services fall under the Substance Abuse Case Management Program umbrella:
- Detoxification Services
- Residential Services
- Supportive Housing Services
- Women’s Recovery Services
Detoxification Services
Detoxification Services provides medically monitored inpatient detox to clients who need medical monitoring oversight during a withdrawal period from alcohol and/or benzodiazepines, etc., and who may have chronic medical conditions. The service focuses on short–term intensive case management for detoxification. Case managers assess individual eligibility status for appropriate placement. Staff is in frequent contact with psychiatric/medical professionals and detox treatment facilities for booking bed space, conducting discharge planning, and meeting client’s aftercare and case management needs. Staff links and refers clients to the Outpatient Program for further treatment at the NCSB.
Specific services under Detoxification Services include:
- Linking individuals to services and supports
- Consulting with hospital medical and social work staff on the diagnosis and behavior of the individual
- Identifying any barriers in linking individuals to the detoxification intake process
- Assisting the individual in meeting eligibility requirements, i.e. picture identification, social security card
- Exploring and assisting individual with avoiding potential relapse
- Developing and maintaining a resource list for targeting population needs within the community
- Transportation services to implement plan of care
- Providing support to assist the individual with resolving crises
- Working closely with other liaisons, case managers, therapist, counselors, and other psychiatric and medical professionals to coordinate needed services
Residential Services
The mission of the Residential Services case manager is to provide short term, intensive case management to individuals in need of residential treatment services and successful discharge from services. The service provides short-term intensive case management services, assuring the success of individuals seeking residential treatment. The case manager will manage the individual’s eligibility status for appropriate placement. Staff is in frequent contact with substance abuse detoxification and residential treatment facilities for booking bed space, conducting discharge planning, and meeting client’s aftercare needs. The case manager also assures appropriate discharge planning and support for the individual successful return to the community. Specific services provided include:
- Linking individuals to services and supports
- Consulting with hospital medical and social work staff on the diagnosis and behavior of the individual
- Identifying any barriers in linking individual to the residential intake process
- Assisting the individual in meeting eligibility requirements, i.e. picture identification, social security card
- Developing and maintaining a resource list for targeted population needs within the community
- Transportation services to support the plan of care
- Providing support to assist the individual with resolving crises
- Working closely with other liaisons, case managers, and resource specialists to coordinate needed services for individuals
Supportive Housing Services
The mission of the Supportive Housing Case Manager is to provide temporary housing SA Hope and intensive case management to individuals who are currently enrolled in Substance Abuse or Co-occurring services.
Adult males and females are provided with temporary housing. Individuals are assisted in finding and maintaining employment so that they can transition to independent housing within three to five months of program enrollment.
During this period clients are also expected to participate in a substance abuse outpatient continuum of care. Individuals learn life skills related to independent living including budgeting, finance, and self-care plus employment referrals.
Specific services provided under the Supportive Housing umbrella include:
- Assessment of case management needs
- Intensive monitoring and linkage to community services
- Individualized education including: budgeting/managing money, independent living skills
- Linkage to (DRS) Department of Rehabilitation Services for employment counseling and job placement
- Use of community resources, especially recovery support group
- Drug-Free Housing
- Transportation services to implement plan of care
- Monitoring of abstinence through drug screening
- Monitoring health compliance and support for health care
- Monitoring attendance and compliance with substance abuse treatment
- Monitoring of quality of service delivery and client satisfaction
- Advocacy
Women’s Recovery Services
This service is designed to provide specialized case management to women who have a substance abuse disorder and case management needs. Outreach services are also conducted by the women’s services case manager. The Women’s Recovery Services program provides:
- Engagement of Temporary Assistance to Needy Families (TANF) clients receiving postpartum referrals from local hospitals
- Inter-agency referrals
- Southeastern Family Project (SEFP) residential treatment services for pregnant women.
Specific services include:
- Outreach presentations within the community
- Comprehensive assessment of case management needs
- Development in conjunction with the individual, an individual service plan (ISP)
- Linkage to (DRS) Department of Rehabilitation Services for employment counseling and job placement
- Referrals for community resources and supports
- Transportation services to support plan of care
- Monitoring of abstinence through drug screening
- Health education and monitoring health compliance and support for health care.
- Linkage to mental health services
- Assistance with applying for benefits
- Assist clients with seamlessly access intake services
- Monitoring of quality of service delivery and client satisfaction
- Advocacy
