The FAPTs in Norfolk are appointed by the Directors of the partner agencies and shall:
Act on utilization review recommendations
Formulate service plans
Monitor case management responsibilities
Monitor prescribed service regiments
Recommend lengths of residential stays
The purpose of the FAPTs shall be to assess the strengths and needs of children and families, determine the complement of services required to meet those needs, develop an Individual Family Service Plan (IFSP) for each child and family, and authorize services. Norfolk has ten teams that meet each month to coordinate the interagency efforts to ensure that families receive the services needed to be successful.
