Intake & Service Coordination

ITC_2From the initial referral, your family will be assigned a Service Coordinator who will be with you throughout your child's enrollment in the program.

Your Service Coordinator will:

  • Be your advocate and will assist you through the entire early intervention process
  • Ensure that your family's rights are protected;
  • Facilitate the evaluation;
  • Make necessary referrals for services and supports;
  • Make contact with you at least monthly;
  • Monitor your child's progress;
  • Provide you with information about community activities and resources;
  • Ensure you are happy with the services we are providing.

The Intake Appointment

All services begin with the initial intake appointment. For detailed information about the appointment, open the document, "What to Expect at your Child's Intake (PDF)." The goal of the intake appointment is to:

  • Discuss your concerns about your child's development
  • Explain the Infant & Toddler Connection of Norfolk services
  • Collect necessary documentation and complete necessary paperwork to proceed to assessment and evaluation
  • Schedule a comprehensive evaluation
